Transitional Living Facility located in Durham, NC is a short-term (up to 30 days) residential setting licensed as a Supervised Living Residence. This service provides care, habilitation and rehabilitation support for individuals having mental illness, developmental disability or substance abuse disorder that requires residential supervision. The purpose of the Transitional Living Facility is to step down clients from crisis services and/or from hospital settings when clinically appropriate to do so. The service is designed to provide transitional support in a supervised residential setting until appropriate long-term residential supports are secured. Active case management support is provided through The Durham Center's Provider Network with the goal of securing appropriate living arrangements in the community as soon as possible. Freedom House works closely and cooperatively with case management providers and participates in the Person-Centered Planning process.
