Support in the Community

Community Intervention Services (CIS) provides mental health and substance abuse interventions necessary to assist an individual in achieving and maintaining Person Centered goals. Services offered include care coordination, case management, skill building, advocacy, and individual and family therapy. Qualified Professionals and licensed clinicians provide services in the community (schools, homes, courts, etc.) at no cost to the client.

The purpose of Community Intervention Support includes:

• Preventing out of home placement
• Linking and referring to community resources and support
• Building relationships with natural supports
• Reducing and stabilizing mental health and substance abuse symptoms
• Modifying behavior
• Developing individualized treatment plans
• Assisting in establishing and maintaining a medical provider
• Increasing one's ability to live independently and manager his/her own care
